Significance of Endothelium Dysfunction in the Development
of Chronic Ischemic Disease of Heart

Summary: The present investigation deal with the study of degree and significance of endothelium dysfunction in the contingent of patients suffering from various functional class (f.c.) chronic ischemic disease of heart (CIDH). Materials and methods: The study was carried out with the 72 CIDH patients, who formed the main group (m.g.). The patients of the m.g. were divided according to the fixed classification. The I f.c. consisted of 17 patients; the II - consisted of 16; and the III and IV consisted of respectively 16 and 18 patients. Control group (c.g.) consisted of 22 practically healthy persons. We judged of the endothelium functional state according to the brachial artery endothelium dependant vasodilatation and the rate of endothelium relaxation factor in blood. According to our investigations there was a decrease of endothelium dependent vasodilatation in the patients of the main group compared to the control one, and the heavier was the case of disease the higher this decrease was expressed. During relatively light case of the disease the breach of endothelium dependant vasodilatation was conditioned mainly thanks to the low level of endothelium relaxation factor, while during the heavy form of the disease (III-IV f.g.) by both its low and high level (respectively 37,5 and 44% of cases). Considering the results of the studies the issue of insertion of the purposeful, but necessary differential therapy in the complex of CIDH treatment arises logically.
